Children May not particepate in the birthday party activities without a waiver completed.

Participant or Guardian agrees that all exercise or programs are undertaken at the participants or Guardianships risk. The participants or Guardian understands the procedures and exercises involved in instruction and participation. They understand that there is a risk of injury involved in the course of instruction and participation and with this knowledge agrees to indemnify and save harmless Hinsadle Fitness club it’s representatives employees or owners from all losses caused by neglect, accident, or injury to the Participant, or by third persons who may be Participants, guest or visitors or under Guardianship i.e. children of guardians participating in the Club programs. In the event that either the participants, Guardian or said third person is injured during the performance and execution of exercise or participation in instruction in any way shape or form. The participants or Guardian takes full responsibility. The participants or Guardian certifies that the participants or Guardians are in good health and not under Doctor’s care and are able to safely participate in the Hinsdale fitness clubs programs and activities.
